PERSONAL HELP FOR SUBSTANCE USE
   When there is an initial consultation requested regarding substance abuse for personal health purposes we will assess during the initial interview your concerns about your substance use, make recommendations and provide outpatient counseling or refer for more intensive counseling. If there is health insurance, we will file the insurance health insurance claim, the patient will make the appropriate health specialist payment at the time of service.

EMPLOYEE REQUEST FOR EVALUATION & RECOMMENDATIONS
   Corporations want to retain executives and valuable employees whose productivity is impaired. When there is an initial consultation for substance abuse requested by your company this requires extensive interview of strengths and functioning, formal assessment protocols, substantial historical information, review of job description and performance, previous reports, collateral interviews, substantially longer interview time to yield a formal written report of recommendations. Health insurance does not cover executive evaluations and reports.

CIVIL LEGAL REQUEST FOR EVALUATION & RECOMMENDATIONS
   When there is a consultation for substance abuse requested by judges, lawyers, or other agencies typically regarding the use that affects parenting or possible Marchman Act, this requires an in-depth evaluation. We typically carve out half a day to interview and administer evaluations. This formal assessment includes protocols such as administration of MAST, DAST, TADD, Binge Drinking Inventory, SASSI and other evaluative tools. Our assessment forms include the history of substances used and current patterns of use. Most evaluations will also include concerns of mental health such as Depression or Anxiety Disorder. The interview includes substantial personal historical information, review of previous reports, collateral interviews, substantially longer interview time, and a formal written report of recommendations. If there is a substance abuse concern we will recommend an appropriate level of care and find a qualified provider. Additionally drug testing is recommended if not already provided. Health insurance does not cover evaluations and reports for civil, criminal, or employment requests.

DOT REQUIRED SAP EVALUATION & RECOMMENDATIONS
   When there is a initial consultation for substance abuse evaluation for DOT purposes this requires: Two face to face meetings and two written reports. (Covid 19 has relaxed interviews to utilize Telehealth) The evaluation is a administration of formal substance abuse assessment protocols, substantial historical information, review of previous reports, collateral interviews, substantially longer interview time and formal written reports of recommendations. Health insurance does not cover this and the employee is responsible for cost.

One: Reason for Testing

Testing positive ( 0.04 and above) on an alcohol test, using alcohol while on duty, refusing to submit to a required alcohol test, Consuming alcohol in the 8 hours following an accident, testing positive for a controlled substance, refusing to submit to a controlled substance test, using any controlled substance without a prescription, and providing a safety sensitive function within four hours of consuming alcohol. Pre-employment, random, post-accident, and for reasonable supicion positives. The employer gives the employee information to contact a SAP.

Two: Refer to a SAP

The SAP then monitors the progress of the employee in the treatment program. When sufficient progress has been made in the recommended program the SAP schedules a return face to face follow up evaluation.

Three: The SAP I Evaluation

A face to face evaluation resulting in recommendations for education and / or counseling. The results and recommendations of this evaluation are given to the employee and sent to the employer.

Four: Employee Education & Counseling Fulfillment

The SAP then monitors the progress of the employee in the treatment program. When sufficient progress has been made in the recommended program the SAP schedules a return face to face fallow up evaluation. Please note only successful completions are recommended to return to work.

Five: The SAP II Evaluation

The SAP meets with the employee regarding a return to work that will include a testing program recommendation. A report is sent to the employer regarding the employee’s successful completion and readiness to return to duty and a specific testing program.

Six: Return to the Employer

The employee returns to meet with the employer regarding return to work instructions and processes required.
